信用卡/郵局紙本授權書捐款
本會目前提供任何一家信用卡及中華郵政每月定期定額或單次捐款，請填寫下載捐款授權書（簽名處要親筆簽名喔！），
並勾選「單次」或是「每月定期」、金額及基本資料，填妥後：
1. 傳真至02-2704-0991，收到傳真後，我們將會立即與您確認喔!!（例假日順延）
2. 郵寄至：10681台北市大安區信義路四段263號3樓之6 -社團法人中華民國保護動物協會 收
★每月(單次／每月定期定額捐款) 扣款時間:
中華郵政 每月5日 扣款。
信 用 卡 每月15日 扣款。
【注意事項】
1.為響應環保，協會已增設電子收據的功能，有提供E-Mail的捐款人，會直接寄送電子收據給您。若沒有E-Mail的捐款人，則寄紙本捐款收據。選擇紙本或電子收據的捐款人，年度收據統一於農曆年前紙本寄發或E-Mail，如有特別情況請來電告知。
2.「社團法人中華民國保護動物協會」為保護您的的個人資料，依據個人資料保護法第8條規定，向台端告知當您填寫此相關資料完成之後，即表示您同意所提供之個人資料將使用在捐款管理及協會相關活動通知之用。
Credit Card/Wire Transfer Authorization Form
Download the authorization form here if you wish to make a one-time or monthly credit card donation. (including wire transfer method via Chunghwa Post account.)
1. Please fax the completed form to 02-2704-0991, APA staff will contact you shortly to confirm receipt of your authorization form (or on the following work day if the form is submitted on a holiday).
2. Please send the completed form to APA’s Taipei office at 3rd Fl-6, No263 Xinyi Road Section 4, Da-an District, Taipei City, Taiwan. (Area Code 10681)
Process Date:
- Wire transfer via Chunghwa Post account – 5th of every month
- Credit card – 15th of every month
Please note:
1. In response to environmental protection, the Association has added the function of e-receipts. For donors who have provided E-Mail, e-receipts will be sent to you directly. For those who do not have E-Mail, paper receipts will be sent to you. For donors who choose paper or e-receipts, annual receipts will be sent in paper form or by E-Mail before the Lunar New Year, please call us to let us know if there are any special circumstances.
Your personal info provided in the authorization form will be used only for donation purposes and may be included in APA’s email distribution list to notify you of any APA-related events/news.
